Percutaneous discectomy has been considered superior to other procedures in the treatment of herniated lumbar disc still being contained by the annulus or posterior longitudinal ligament. From Nov. 1998 to June 1993, a total of 42 patients treated by Onik’s and Kambin’s percutaneous discectomy, alternatively at 50 levels, and followed-up between 8 months and 4 years 7 months (average 2 year 2 months) showed an overall success rate of 71.4%. In 1998, we first adopted the percutaneous discectomy procedure using the Onik’s selection criteria and methods. The initial results of 15 cases, which had a 66.7% success rate, were not satisfactory in comparison with previous reports. Since then, we performed Disco CT pre-operatively as a routine procedure in order to more carefully select candidates, which resulted in the following 27 patients showing a 74% success rate. In a subgroup of 22 private pay patients, the success rate was 81.8%. In these series, there was no significant difference between the parameters of disc level, amount of disc material removed, patient’s gender and treatment technique. No intraoperative or postoperative complications occurred. In conclusion, the result of our series could be comparable to other reports. Likewise the advantages of a low morbidity, and easily performed through a percutaneous approach make it an attractive alternative to a more invasive open procedure in the treatment of herniated lumbar disc. One must be cautioned however, to carefully select patients in order to ensure not only safe but effective results.
