Fractures of Tibial Pilon-Surgical Experience of 40 Cases

Forty tibial pilon fractures were categorized according to Mast and Spiegel’s clas-sification. Ten fractures were in Type I, 8 in Type II and 22 in Type III. In Type I and Type II fractures, anatomic reduction and internal fixation could be achieved as possi-ble and satisfactory results, were obtained in 83% of these patients. However, the Type III fracture was a challenge to management and the satisfactory result was only 55%. In order to achieve satisfactory surgical results, we considered the following factors were important: (1) restoration of fibular length (2) do every effort to restore the articular surface as possible (3) maintain distal tibial length by plate or external fixator (4) bone grafting the metaphyseal defect if necessary (5) delicately manage-ment in soft tissue.
