From June 1986 to June 1992, forty-two aged patients received surgical manage-ment for lumbar disc herniation. Twenty-three were male, and 19 were female. The age ranged from 60 to 78 years (mean, 66 years). The main clinical manifestation was sciatica with some neurological deficit. In the patients with acute sciatica, root pain was the dominant manifestation which was sensitive to the straight leg raising test. In the patients with chronic sciatica, numbness and paresthesia were dominant and were insensitive to the straight leg raising test. Twelve patients received decompressive laminectomy in addition to discotomy for the advanced degenerative changes. Rup-tured discs were found in 21 patients. The pathology of 32 discs revealed fibrocarti-laginous degeneration. Two rupture discs showed myxomatous degeneration. Totally, thiry-six could be followed-up and the other six could be contacted with the telephone. Totally, 33 (78.5%) got excellent or good surgical outcome.
