Transpedicle screw instrumentation is a trend of surgical treatment of spondylolisthesis. However, pedicle screw insertion is not without risks. Neurologic injury due to inaccurate pedicle screw insertion is possible. Between July 1990 and December 1992, 280 patients of spondylolisthesis were operated with laminectomy, posterolateral fusion and pedicle screw instrumentation. Seven patients experienced unexpected root pain of lower limb after operation and were successfully diagnosed to be caused by misplaced screw from lum-bosacralplain radiographs and CT-scan study. The radiculopathy were all dra-matically relieved by removal of screws from one to 26 days after the previous operation.
