From 1987 to 1991, total 443 AML THR were performed in our hospital. There were twenty-one (4.7%) prostheses revised due to severe osteolysis. Three male and eighteen female were included. The average age was 49.4 years old. The average cup size was 48.3 mm and most hips used 32 mm head. The average cup inclination was 52.1 degree. We could find periprosthetic osteolysis in acetabulum (10%), femur (23%), and both (67%) respectively. Eleven cups, 3stems and 7 both components were revised. The average survival of prosthesis was 74.5 months. Eighteen patients were followed. Using Harris hip score, 15 hips showed good, 2 hips showed fair and one hip showed poor. In this study, younger age, vertical cup inclination, acetabular dysplasia, larger head with smaller cup revealed the increased possibility of osteolysis and revision rate.
