Necrotizing Fasciitis after Bilateral Hip Arthroplasty - A Case Report

兩側人工髖關節置換術後發生壞死性肌膜炎之病例報告

並列摘要

Necrotizing fasciitis is a rapidly progressive, severe bacterial infection involving the superficial layers of the extremities, abdomen, or perineum. Necrotizing fasciiis often begins with trauma that can be as innocuous as a sim-ple contusion, insect bite, or minor burn. A case of bilateral femoral avascular necrosis was identified 38-year-old male patient who had bilateral hip arthro-plasty. Group A streptococcal necrotizing fasciitis developed around the patient’s bilateral surgical scars after he’d received a minor abrasion in seawa-ter. Rapid onset of sepsis, adult respiratory distress syndrome (ARDS) and mul-tiple organ failure ensued. Unfortunately, this young patient died 7 days after being infected. Differentiating necrotizing fasciitis from common soft-tissue infections, such as celllulitis and impetigo, is both challenging and critically important.
