Treatment of Humeral Intercondylar Fracture with Triceps-Reflecting Anconeus Pedicle (TRAP) Approach

Treatment for humeral intercondylar fracture is a challenge to many ortho-pedic surgenons, not only for the intra-articular character of the humerus frac-ture, but also because of the need to reduce the humerus anatomically. The most crucial step for achiveving successful treatment is exposing the distal humerus. Many described approaches include: triceps-splitting, triceps-reflecting, triceps-preserving, and transosseous approaches. From December 2000 to Feb 2001, three cases of complex distal humeral fracture (AO classification C) were treat-ed in Far Eastern Memorial Hospital by the triceps-reflecting anconeus pedicle (TRAP) approach. Monthly follow-up radiological evaluation and functional evaluation using the Mayo elbow performance score were performed. All patients achieved functional bone union with satisfactory range of motion three months later. And their functional scores were all above 80, and were graded as good. The advantages of triceps-reflecting anconeus pedicle (TRAP) approach in the treatment of humeral intercondylar fractures include: Providing adequate exposure, soft tissue dissection without osteotomy, rapid rehabilitation of the involved part, reduced hardware complications and nonunion at the site of ole-cranon osteotomy, and less risk of contributing to posttraumatic arthritis or intra-articular adhesions. From our experience, triceps-reflecting anconeus pedicle (TRAP) approach is a good alternative to olecranon osteotomy for the treatment of humeral intercondylar fractures.
