Failure of the polyethylene liner after uncemented anatomic medullary locking (AML) total hip arthroplasty with acetabular cup system (ACS) was reported in the literature. Catastrophic wear of the metallic acetabular component is extremely uncommon. We report a case that occurred ten years after implantation. Improper design is considered to be the cause of excessive wear of the polyethylene linear followed by wear through of its metal backing.
