Vacuum-Assisted Closure System Used in the Management of Complicated Orthopedic Wounds

Devascularized wounds, severe trauma with significant or extensive soft tissue defect, bacterial infection of the bone and/or joint after surgery or trauma with wound necrosis, reluctant stasis ulcers and wounds in immunocompromized patients are difficult to manage and present complicated challenges to the orthopaedic surgeon. From November 2002 to February 2004, we used a vacuum-assisted closure (VAC) system to treat complicated orthopaedic wounds such as these in 22 patients. With the vacuum effect, we were able to decrease tissue bacterial levels effectively and rapidly and also promote local vascularization and granulation tissue formation. Successful application of VAC technique enabled us to control infection and achieve coverage of these complicated wounds. Most of these wounds could be covered with split-thickness skin grafts placed onto the abundant, fleshy granulation tissue from the VAC effect. Other wounds could be closed primarily or by using a regional rotational flap. Modifications of the VAC technique were needed to address the variety of complicated wounds. We offer our innovations to resolve the problems we encountered in applying the VAC technique to these complicated wounds.
