Loss of Vision after Spine Surgery: A Case Report and Risk Factors Analysis

The incidence of postoperative visual loss is about 0.2%, but it is more frequent in longer and more complicated surgical procedures. We report our experience with a 58-year-old man who was treated for degenerative spinal stenosis from the L4 to S1 level with posterior decompression, posterior-lateral fusion, and pedicle screw fixation. He was found to have loss of vision in his right eye immediately after surgery. A series of ophthalmologic studies revealed central retinal artery occlusion to be the underlying cause of this unilateral, eventually permanent, complication. A review of factors that can contribute to postoperative visual loss show there are many factors on the part of the patient, anesthetist, and surgeon that can contribute to such a tragedy. In order to avoid such a complication and manage patients who present with postoperative visual loss, clinicians must be aware of predisposing factors and their interactions.
