Comparison of Intracapsular Femoral Neck and Intertrochanteric Fractures: Surgical Outcomes and Quality of Life in Elderly Patients

Purpose: Intracapsular femoral neck and intertrochanteric fractures in the elderly are common. While favored treatment methods for both fractures have been largely defined, the relative prognosis of both types of fractures has not been compared despite advancement of medical care and modern technology. Methods: Forty-one intracapsular femoral neck and 57 intertrochanteric fractures in consecutive elderly patients to our institution were treated with surgery. Prospective comparison of all relevant data of patients, including admission, discharge, and postoperative follow-up course of 1, 3, 6, and 12 months, was conducted in patients with either type of fracture. In addition to conventional clinical measures, health-related quality of life including activity of living (ADL ), instrumental activity of living (IADL ), and short form-36 (SF -36) were assessed. Results: On admission, patients with intertrochanteric fractures were significantly older than patients with femoral neck fractures (81.3 vs. 77.7 years, P=0.019). Upon regular follow up, ADL, IADL, and nearly all SF -36 evaluations were similar in both groups. Only physical activity at 12 months was better in the trochanteric group than the femoral neck group (P=0.048). Conclusions: The prognosis for both fractures after surgical treatment is similar. Although surgical treatment can achieve a high success rate, one-year mortality is still high (11.2%). Furthermore, patients' health-related quality of life did not reflect a recovery to pre-injury status even after 1 year.
