富含n-3多元不飽和脂肪酸之藻油補充品對於使用化放療頭頸癌患者之影響-病例報告

The Effect of n-3 PUFA-rich Algal Oil Supplementation in a Head and Neck Cancer Patient During CCRT: Case Report

摘要


放射線與化療是常見之頭頸癌療程之一,放化療期間增加n-3多元不飽和脂肪酸(n-3 poly-unsaturated fatty acids, n-3 PUFAs)之攝取,可以幫助降低體內之發炎反應,素食來源之藻油富含n-3多元不飽和脂肪酸,本篇將討論藻油對接受合併化放療頭頸癌病患的影響。本個案為1位59歲女性,鼻咽癌第2期(cT1N1M0, stage II),接受放療合併化學治療。個案茹素多年。在營養師營養介入前,個案之白蛋白及C-反應蛋白指數皆屬於正常值間。個案因即將放化療,故給予化療期間營養注意事項衛教及n-3多元不飽和脂肪酸對於治療期間之影響,且願意使用藻油補充品。經營養介入後,個案之白蛋白指數維持正常,C-反應蛋白上升減少,減輕之體重小於文獻中之10%,顯示藻油能維持頭頸癌病患之整體營養狀況。

Radiotherapy and chemotherapy are common head and neck cancer regimens. Increasing the intake of n-3 polyunsaturated fatty acids (PUFAs) during radiotherapy and chemotherapy can reduce inflammation. Algal oil is a vegetarian product and is rich in n-3 PUFAs. We discuss the effect of algal oil supplementation in head and neck cancer patients during [?]continuous renal replacement therapy (CRRT). This case was a 59-year-old woman with stage II nasopharyngeal carcinoma (NPC) receiving radiotherapy and chemotherapy. She had been a vegetarian for many years. Before the dietitian's nutrition intervention, albumin and C-reactive protein (CRP) were in normal ranges. Before radiotherapy and chemotherapy, she agreed to receive nutritional education about the effects of n-3 PUFAs during chemotherapy and used a vegetarian source of an n-3 PUFA supplement. Following a sustained nutritional intervention, albumin remained in a normal range, the increase in CRP was reduced, and weight loss was less than 10% that described in the literature. This case report indicates that the algal oil supplementation can maintain the nutritional status in head and neck cancer patients during CRRT.
