Correlation of Intraplatelet Calcium Concentration and PH with Kt/V and Blood Pressure in Maintenance Hemodialysis Patients

The aim of this study was to investigate the correlation between Kt/v, platelet intracellular calcium concentration (〔Ca²+〕i), intracellular pH (pHi) and blood pressure in maintenance hemodialysis (HD) patients. Platelet 〔Ca²+〕iand pHi were measured by fluorescent method using Fura-2 and BCECF, respectively. Platelet 〔Ca²+〕i was significantly higher (106 ± 26 vs 87 ± 18 nmole/L, p<0.01) and pHi was significantly lower (7.11 ± 0.06 vs 7.15 ± 0.04, p<0.01) in uremic patients than in the control subjects. Platelet 〔Ca²+〕i showed significant positive correlation (r=0.587, p<0.02) to mean arterial pressure (MAP) and strong negative correlation (r=-0.77,p<0.001) to Kt/V, but no significant correlation to pHi. Platelet pHi did not have any significant correlation to MAP to kt/V. however, Kt/V showed significant negative correlation (r=-0.574,p<0.009) to MAP. Patients with Kt/V≥1.4 had significantly lowere platelet 〔Ca²+〕i (88 ± 19 vs 126 ± 17 nmol/L,p<0.01) and lower MAP (91 ± 9 vs 110 ± 12 mmHg,p<0.01) than did the patients with Kt/V<1.4. In conclusion, these results demonstrate the close interrelationship between platelet 〔Ca²+〕i, MAP and Kt/V in uremic patients on maintenance HD. Patients with higher doses of dialysis have lower platelet 〔Ca²+〕i and lower MAP before HD treatment.
