Acute Renal Failure after 5-Aminosalicylic Acid Therapy in a 47-Year-Old Woman with Ulcerative Colitis

Inflammatory bowel disease such as ulcerative colitis and Crohn's disease is frequent treated first line by 5-aminosalicylic acid (5-ASA). Acute renal failure due to acute interstitial nephritis is an infrequent complication of 5-ASA therapy. In most reported cases of 5-ASA-induced interstitial nephritis, patients had taken the drug for 6 months or more before significant renal insufficiency was documented. We described a 47-year-old female with ulcerative colitis who suffered from symptomatic acute renal failure (BUN and serum creatinine were 51mg/dL and 5.2mg/dL) after one year 5-ASA therapy. Renal biopsy revealed acute interstitial nephritis. After discontinuation of 5-aminosalicylic acid, renal function returned to baseline four months later (BUN and serum creatinine are 26mg/dL and 1.1mg/dL).
