發展偵測缺氧修飾白蛋白之自動化檢測法

Establishment of an Automated Method for Detecting Ischemia Modified Albumin

摘要


缺氧修飾白蛋白(ischemia modified albumin, IMA)可以早期偵測心肌缺氧,在臨床上的應用爲排除急診胸痛病人罹患心肌缺氧性心臟病發的可能性。目前市面上由Inverness Medical Professional Diagnostics公司出產的ACB® test (Albumin Cobalt Binding Test),可用來偵測血清中IMA的量,其價格昂貴且開封後效期短,因此本實驗室決定自行研發in-house ACB test以供臨床研究之用。本實驗建立的自動化檢測法,其偵測範圍從39.6到214.6U/mL,低值與高值品管液的within-run與between-run的不精密度皆小於5%。在正常參考值建立上,因爲血清檢體測出的IMA量較血漿中的量低約18%,所以已分別建立血清與血漿的參考值各爲108.8與120U/mL,此外,不同性別與不同年齡層間的IMA並無明顯差異。我們分析了28位急性心肌梗塞的病人,到急診時其血漿中的IMA量明顯高於正常人。以上結果顯示本實驗室成功建立了in-house ACB test,可應用於血液中IMA的偵測,並且未來可提供臨床研究用途。

Myocardial Ischemia can be detected early by elevated ischemia modified albumin (IMA) in order to rule out the possibility of having an ischemic heart attack. ACB® test (Albumin Cobalt Binding test), which manufactured by Inverness Medical Professional Diagnostics, Inc., can be used to measure the value of IMA in serum. Due to its drawbacks of high price and short expiry date, we decided to develop an in-house ACB test for research use. The detection range of this in-house assay spanned from 39.6 to 214.6 U/mL. In addition, both the within-run and between-run imprecision were less than 5%. In establishing the reference values, the IMA level in serum was 18% lower than that of plasma; therefore, the reference values set up for serum and plasma were 108.8 and 120 U/mL respectively. Furthermore, there is no distinction between gender and age in the detection of IMA. 28 patients suffered from acute myocardial ischemia were studied, their IMA in plasma is higher than that of normal person. The result above reveals that we have successfully established an in-house ACB test, which can be applied in the detection of IMA and also can be used for future research purpose.
