利用多套組即時聚合酵素連鎖反應鑑別診斷多種呼吸道病毒感染

Differential Diagnosis of Multiple Viral Respiratory Pathogens by Multiplex Real-Time PCR

摘要


我們發展設計出多套組即時聚合酵素連鎖反應技術,在同一檢體中能同時鑑定九種常引起急性呼吸道感染的病毒,包括流行性感冒病毒A型與B型、呼吸道融合病毒、副流行性感冒病毒1、2、3型、鼻病毒、腺病毒及肺炎黴漿菌。我們收集240名成人具有類流感症狀的咽喉拭子檢體,利用此技術進行病毒篩檢,並與病毒培養結果比較。結果顯示,利用此技術篩檢到58件檢體爲病毒陽性,病毒鑑定陽性率爲24.7%,而病毒培養陽性率僅5.1%,因此,採用此技術可大幅提高病毒檢測的敏感度,尤其對於鼻病毒及黴漿菌的檢測有明顯的助益。

Objective: A multiple viral respiratory pathogens detection system based on multiplex real-time polymerase chain reaction (PCR) was set up in this study. Nine common respiratory tract infectious agents including Influenza A virus, Influenza B virus, Respiratory Syncytial virus, Parainfluenza virus type 1 to type 3, Rhinovirus, Mycoplasma pneumonia, and Adenovirus were detected in one specimen by four sets of multiplex real-time PCR. Totally 240 throat swab specimens collected from adults with flu like symptoms were detected both by the multiplex real time PCR system and conventional virus isolation. The results revealed that 58 specimens showed positive results by the real-time PCR system, the positive rate was 24.7 %. However, the positive rate was only 5.1 % by the conventional virus isolation system. In conclusion, applying the multiplex real-time PCR detection system on clinical diagnosis of respiratory tract infections, especially to Rhinovirus and Mycoplasma pneumonia, will greatly increase the clinical sensitivity.
