精神分裂症患者空間關係概念之探討

摘要

本研究旨在探討精神分裂症病患職能治療前後及出院前等三階段，其空間關係、雙手組合功能、及工作能力三者間的關係。研究對象爲民國74年7月至75年3月間住過臺北市立療養院之精神分裂症病患，住院期間曾接受職能治療三周以上，年齡在20成至49歲之間、教育程度爲國中或以上，曾接受三次職能評估及出院三個月後追蹤者，共15位。
研究結果顯示：(1)初期和隔三周評估時，病患在空間關係測驗之速度和普渡手功測驗之組合分項成績，在統計上呈現著相關。(2)出院前評估時，病患之判斷空間速度、精確度典普渡手功能測驗之組合分項成績，彼此間有顯著相關。(3)三次職能評估在判斷空間關係之速度和精確度上，除了初期評估之速度外，均與常模組無顯著差異。(4)空間關係測驗之速度和精確度在三次職能評估上分別呈顯著差異。(5)出院前病患在空間關係測驗之速度和精確度與普渡手功能測驗之組合分項成績及從事職能活動時之工作行爲類總分與其出院後之就業狀況均無顯著相關。

The purpose of this study was undertaken to investigate the relationship among the spatial relations abilities, assembly function and work abilities during three times occupational evaluation (at the beginning, 3 weeks later and before discharge). The subjects, age from 21 to 49, consist of 15 schizophrenics hospitalized at Taipei City Psychiatric Center during the period from July, 1, 1985 to March 15, 1986. The 3-month follow-up study of them was performed.
Results showed that there was significant correlation between the velocity scores of the MSRT and assembly scores of the PPMT at the first and the second occupational evaluation. But there were no significant correlations on the MSRT scors, scores of task behavior on the COTES-R at the first and the second occupational evaluation. At the third occupational evaluation, there were high correl-ations on the velocity and accuracy scores of the MSRT, and assembly scores of the PPMT.
As to the MSRT scores, there were significant differences among three times occupational evaluation. There was a significant difference on the velocity scores of the MSRT between the subject group and the normal group at the first occupational evaluation, but there were no significant differences after subjects got treatment. There were no significant differences on assembly scores of the PPMT, and scores of task behavior on the COTES-R among three times occupational evaluation. There were no significant correlations among the MSRT scores, assembly scores of the PPMT, scores of task behavior on the COTES-R before discharge, and the occupational status at the 3-month follow-up after discharge.
