中風病人早期患側上肢動作能力與半年後手部肌力的相關性

The Relationship between Early Motor Status and Hand Strength after Six Months in Stroke Patients

摘要


在中風病人患側上肢的恢復過程中,軟弱期(flaccidity)的出現與否及長短是決定病人手部功能預後的重要因素。本研究發現中風病人在發病一個月後其動作能力若還停留在Brunnstrom動作等級第二級以下,則其半年後的手部握力與揑力的缺失將分別達89%及84.4%。但發病一個月後,其動作能力若達Brunnstrom動作等級第三級以上,則半年後手部握力與揑力的缺失約為38.8%與29.6%。 在中風病人的動作恢復過程中,雖然早期握力的動作常較揑力的動作較早恢復。但在半年後揑力的恢復程度卻比握力好,並有顯著的差異性。

The presence and duration of flaccidity often play an important role in determining the prognosis of the function of involved hand during motor recovery period. The result of this study was shown, that the grip and pinch strength deficits after six months would reach 89% and 84.4% respectively, if the motor stage of the affected upper limb remained at or below Brunnstrom motor stage Ⅱ one month after stroke. The grip and pinch strength deficits after six months would only reach 38.8% and 29.6% respectively, if the motor stage of the affected upper limb remained at or above Brunnstrom motor stage Ⅲ one month after stroke. Although the grasp function was thought to have an earlier restoration than pinch function, this study revealed the recovery of pinch strength was better than grip strength after six months.
