中風偏癱病人動作恢復與姿勢穩定之關係探討

Relationship between Motor Status and Postural Stability in Stroke Patients

本篇文章主要探討中風病人在不同的下肢動作恢復時期(Brunnstrum's stage,以下簡稱Br. Stage),其姿勢穩定能力是否有所不同。實驗對象為兩組下肢動作恢復分別在Br. Stage 5-6及Br. stage 3-4之右邊中風偏癱病患,另有一組年齡相配之正常人作為對照組。實驗方法為利用姿勢測量儀器(Balance Master System & Software version 2.20)測量(1)病人與正常人雙腳之承重百分比;(2)在三種不同視覺狀況(張眼、閉眼、視覺迴饋)下,重心擺動之大小。結果顯示:在雙腳承重百分比方面,Br. Stage 5-6組之對稱情形與正常對照組幾乎無異,但Br. Stage 3-4組則明顯不對稱。此結果顯示:中風病人患側動作能力恢復良好者之雙腳承重對稱性可能較恢復程度不良者要理想。另外,在擺動大小之方面,兩組中風偏癱組之間無統計學上顯著差異存在;中風組與正常對照組之間則具有顯著差異,但在視覺迴饋狀況下,Br. Stage 5-6組與正常組之間與顯著差異,Br. Stage 3-4組與正常組之間雖具顯著差異,但p值小於0.05而已。此結果顯示中風病人承重的對稱性與重心擺動大小無一致性。

The purpose of this study is to investigate whether different recovery stage in hemiplegia will affect postural stability. The motor recovery stage was based on Brunnstrum's recovery stage, and postural stability was recorded as the percentage of weight bearing on affected side and the path of sway area. The hemiplegic subjects are divided into two groups (Br. Stage 3-4 and Br. Stage 5-6), and there is one controlled group (matched elderly person). A significant difference was found between two groups of hemiplegia in the percentage of weight bearing; but no significant difference in sway area. These results reflect that the symmetrical weight bearing in hemiplegia was not consistent with postural sway.
