The Influence of Head-Neck Rotations on the Muscle Strength of Elbow Flexors and Elbow Extensors

頭頸轉動對手肘屈肌與伸肌肌肉力量的影響

本研究的目的在探討頭頸的轉動位置是否影響手肘屈肌與伸肌的肌肉力量。十六位健康成年男性受試者(年齡由19到27歲)參與此項實驗。每位受試者以CYBEX 350等速肌力測試儀進行肘關節屈肌與伸肌的等速肌力測驗。施測時,受試者的頭頸部固定在三個規定的方向(左轉、正中、右轉),以兩個速度(60度/秒、180度/秒)各作三次手肘伸直、彎屈的等速肌力測驗。頭頸位置的順序與角速度的順序均經過隨機處理。結果顯示右手肘屈肌的尖峰力矩值在頭頸左轉的位置大於右轉。雖然右手肘屈肌的尖峰力矩值在左轉與正中、正中與右轉是在預期的大小順序,但他們之間的差別並無統計意義。右手肘伸肌的尖峰力矩值在頭頸位置間並無統計差別。結果似乎顯示頭頸轉向左邊可以增強右手肘屈肌的尖峰力矩值。但由於本實驗的一些限制,結果的臨床意義並不明確。作者在文中對未來的研究方向提供了些建議。

The purpose of this study was to investigate whether the muscle strength of elbow flexors and elbow extensors would be influenced by head/neck rotations. Sixteen healthy male students between the ages of 19 and 27 years volunteered as subjects. Muscle strength was measured by peak torque value. On a Cybex 350 Isokinetic Dynamometer, each subject performed three trials of elbow extension and elbow flexion at two different velocities (60deg/sec, 180 deg/sec) while keeping his head at midlilne, right turn, and left turn positions. The order of head positions and velocities was counterbalanced across the subjects. Results revealed a significant difference of peak torque of elbow flexors between left turn and right turn positions, with no position by velocity interaction. The differences in peak torque between left turn and midline, and between midline and right turn positions were in the predicted direction, although they failed to reach statistical significance. Muscle strength of elbow extensors, on the other hand, was not affected by head positions. The result seemed to suggest that one would gain advantage in the muscle strength of elbow flexors by assuming the aymmetric tonic neck reflex pattern. However, the actual clinical implication is limited due to the limitations of the study. Futher studies of combining Cybex, EMG, and video are recommended.
