摘要


物理治療師了解脊椎炎患者的脊椎活動度對於評估患者之嚴重程度相當重要。四十六位脊椎炎患者和四十六位年齡、性別、體重、身高相仿的健康者參與研究。利用腰椎活動測量儀(LMM)分析脊椎立體動作,立體動作包括:矢狀面、冠狀面和水平面的角度、速度和加速度。受測者皆接受腰椎活動測量儀(LMM)之檢查並得知脊椎活動之數值,利用T檢定(student-tests)比較脊椎炎患者和健康者間的脊椎活動度差別。結果發現脊椎炎患者比健康者有更多矢狀面活動度。脊椎炎患者之脊椎活動度的速度低於健康者。另外對所有脊椎炎患者分析其年齡、性別、體重、和身高四個因素對脊椎活動度的影響。利用逐步迴歸分析(step-wise linear regression)得出影響因素後再利用變異數分析(analysis of variance)得出其影響程度。在四個影響其脊椎活動度的因素分析中,以年齡最為重要:其脊椎活動度隨年齡增加而遞減。另外性別和體重也有明顯影響,男性有較大的脊椎活動度,體重對脊椎活動度之影響程度則須進一步分析。然而身高並不是脊椎炎患者其脊椎活動度明顯的影響因素。文中並分別探討其可能的原因。未來之研究將進一步瞭解不同的脊椎病變之脊椎活動度,並且探討其影響的因素。

Knowledge of lumbar motion in spondylitis patients was important to the physical therapist in assessing levels of spine pathology. Fourty-six spondylitis patients and age-, gender-, body weight-, and body height- matched alike 46 healthy subjects were studied. The lumbar motion monitor (LMM) was used to analyze 3-D spine motion, including range of motion, velocity, and acceleration of sagittal, frontal, and transverse planes which were compared using student t-tests statistically between spondylitis patients and healthy subjects. All the spondylitis patients LMM 3-D spine mobility data were also analyzed by 4 factors including age, gender, body weight, and body height. The step-wise linear regression and analysis of variance (ANOVA) with Scheffe post-hoc analysis were calculated. The results showed that the spondylitis patients had more spine sagittal plane mobility than the healthy subjects. The spondylitis patients had less velocity in spine mobility than the healthy subjects. The order of influencing spine mobility factors were age, gender, and body weight. The body height did not show significant factor. The reasons of the above results were discussed. The spine mobility in spondylitis patients were also presented. Futher studies should emphasize on how different spine pathological condition affected spine mobility and factors that affected the spine mobility.
