摘要

腦中風佔國人十大死因之第二位，也是神經物理治療最主要的服務對象。中風發病後約l/3死亡，約40%發生功能障礙而需要物理治療等之復健服務。本文為神經醫學會之神經復健研討會之演講內容，主要目的在綜論神經物理治療之原理及腦中風物理治療之評估與治療原則，並回顧物理治療之效益。由文獻回顧可知，神經物理治療的評估與治療的方法隨著學理的發展而日新月異。在評估方面，本文介紹了動作分級之判斷標準及一種新的步行功能量表供同業參考。在療效的文獻回顧方面，本文提出了關於促進技術、平衡訓練、步態訓練、以及電刺激治療對於腦中風病人療效之支持性證據，顯示物理治療對於病人的預後有正面的效益。本文希望能促進醫師對腦中風物理治療內容的認識以增進專業間的團隊合作，並供物理治療人員繼續教育之參考。

Cerebrovascular accidents, stroke, is the second leading cause of death in this country. Approximately 40% of stroke victims survive with impairments that require services provided by rehabilitation specialists including physical therapists. Yet many stroke patients were not referred to physical therapy timely from the neurologists. Lack of exposure to rehabilitation concept during residence training for neurologists may be one important cause of the delay in referral to physical therapy. This article was presented at the first neurological rehabilitation training course organized by the Taiwan Neurological Society, Taiwan Stroke Society, and the Physical Therapy Association of the Republic of China on February 22, 1998. The contents of this article included: (1) a discussion of the benefit of physical therapy for stroke patients of different ages and pathologies, (2) the theories underlying neurological physical therapy, (3) current concept of physical therapy evaluation for stroke patients, and (4) effects of specific treatment techniques. Effects of facilitation, balance training, ambulation training, and electrical stimulation for stroke patients were reviewed. Definitions of motor function grades were provided to facilitate accurate communication among professionals.
