臨床路徑架構在物理治療的應用

臨床路徑是醫療管理的工具，它明白揭示病患在該項醫療計劃中將達成的目標，協調執行業務的醫療成員使用一致且最理想的處理程式，在預定的時問內，以最佳效率來達成目標，強調品質與效率。適用於高病患流通率、高醫療成本、但醫療處置步驟變異較少的病況。確立標準化的物理治療病歷紀錄並呈現功能結果是物理治療師因應臨床路徑首要的課題。少部份脫離臨床路程或高度複雜的個素則需個案管理師來做監控，包括與相關專業討論如何調整醫療內容，或重新設定醫療計劃。臨床路徑策略與品質促進的活動事實上有很多的相似點，如過程的揀選、描述、評測和改善。除了著重行政流程效率之外，更可藉著臨床路徑的各項指標來規劃品質管理的活動，提昇醫療工作的品管。物理治療從業人員應開始熟稔臨床路徑，考量本國病人的特性和療程，為一些變異較少的病況訂出合理的治療步驟與流程、預期目標、治療次數與治療期間，並以此做為品質管理的指標。本文最後提出臨床路徑在物理治療應用之一例，也就是冠狀動脈繞道手術後患者住院期間物理治療臨床路徑的建構實例，以供讀者參考。

Clinical path has been applied in health care services since 1980s. It was reported to reduce length of hospital stay, patient charges, and complications. Furthermore, it was used as a tool of outcome management especially involving multidisciplinary patient care program. This article reviewed the history, definition, design, implementation, and procedures of establishing a clinical path. The interface with physical therapy documentation and quality improvement was discussed. In addition, it introduced the role of case manager in clinical path. Finally, an example of clinical path in acute physical therapy for coronary artery bypass graft surgical inpatients was provided. In the era of managed care, physical therapists should be familiarized with these concepts and applications in physical therapy.
