Free Fillet Flap Reconstruction for Large Soft Tissue Defect Following Disarticulation of Hip Joint for Fibrosarcoma

摘要

當惡性腫瘤位於下肢近端，常需要考慮進行髖關節離斷手術以完成廣泛性切除。腫瘤的切除及髖關節離斷手術留下的大範圍軟組織缺失可利用從截肢肢體取下的游離菲力皮瓣以提供良好血循的組織來覆蓋關節離斷術後的大型傷口，而不需額外的皮瓣供應區。在此報告一位57歲男性患者因右大腿後側有一復發性纖維肉瘤，而接受右側髖關節離斷手術，並從右小腿取得游離菲力皮辦以關閉傷口，術後傷口癒合良好。三個月後，患者可穿戴客制化下肢輔具並依靠助行器自行行走。

When a malignant tumor is located at proximal lower limb, hip joint disarticulation must be considered. Free fillet flap harvested from the amputated limb provides an excellent source of vascularized tissues for reconstruction of the large soft tissue defect following tumor resection by means of hip joint disarticulation without additional donor site. We report a 57- year-old male patient who suffered from recurrent right thigh fibrosarcoma. Right hip disarticulation with free fillet flap reconstruction was performed, and the wound healed well. Three months later, a custom-made lower limb prosthesis was fitted successfully, and the patient could then walk with the assistance of a walker.
