Pharmacy interventions over a 6-month period were analyzed in five categories: drug selection, therapeutic outcome monitoring, dose or dosing interval recommendation, dosage form selection and others. In this study, 576 patient-visits d 307 interventions of pharmacy services were examined. The most frequent pharmacy intervention was drug selection (47.6%). Among the total interventions of each patient-visit, 60% were a single intervention, 32.5% included two interventions and 6.8% included three interventions. Overall, the acceptance rate of pharmacy interventions by physicians was 89%. These results suggest that the quality of patient care can be improved through the clinical pharmacy services.
