The fundamental causes determining the failure of valve used for transmission of liquid oxygen in oxygen plant. Visual inspection, SEM/EDS, XRD, oxalic acid etch test, oil extraction technique and oil composition analysis were undertaken determine the causes of the failed valve. The failure of the valve was found to be attributable to carbon contamination. The carbon contamination was susceptible to ignition and burn in the presence of pure oxygen. Subsequently, the oxidation at elevated temperature of the valve occurred. Also, Recommendations for prevention are also suggested.
