Primary cardiac myxosarcoma is a rare primary cardiac malignant tumor. Recurrence demonstrated by imaging findings is hardly found on literature. A 40-year-old male with cardiac myxosarcoma had recurrent tumor after first operation and received second operation, which revealed recurrence. Echocardiography demonstrated a mass attached to the posterior left atrial wall protruding into the mitral orifice during diastole. Magnetic resonance imaging was performed 2 weeks after the second operation and revealed recurrent tumors near the orifice of inferior left pulmonary vein and left posterior wall of the left atrium. Because the echogenicity and signal intensity of the tumors are similar to that of myocardium, serial follow-up of changes of thickness of cardiac is a more reliable indicator of recurrence.
