Impact of Age on the Urodynamic Parameters in the Patients with Symptomatic Bladder Outlet Obstruction

年齡對具有膀胱外阻塞症狀病人之尿路機能的影響

摘要

為了要瞭解在具有膀胱外阻塞症狀病人，年齡是否會影響尿路機能的表現，我們收集了418位具有嚴重排尿障礙症狀的病人。這些病人並無尿滯留或神經性排尿障礙，在經泌尿科醫師診斷具有前列腺肥大或膀胱功能失調，即將接受前列腺手術或是藥物治療。病人的IPSS症狀積分超過15分，這些病人均經過經直腸超音波檢查其前列腺容量。每一位病人都有接受尿流速測量，但膀胱壓力圖僅在348位檢查，壓力/尿流研究有323位，尿道壓力圖有202位病人接受檢查。這此病人再經區分為5個年齡層，由小於45歲到大於75歲每10歲為一年齡層。尿路機能檢查值在不同年齡層做比較，並以迴歸統計方法進一步加以分析年齡與尿路機能檢查的關總。
本研究結果顯示年齡對於尿路機能檢查並無統計的意義。IPSS積分平均為23±9.3，前列腺容量為31.7±15.5 Ml，與年齡的大小均無有意義的相關。在323位進行壓力/尿流研究的病人中，22(6.8%)具有高壓力高流速阻塞，38(11.8%)具有低壓力低流速阻塞，而逼尿肌不隱定則存在於32位病人(10%)。在迴歸分析中，年齡增加最大尿流速會有一緩慢減少的趨勢，但未達到統計意義，而其原因可能系因年齡增加導致的膀胱容量略為減少。在修正後的最大尿流速中並無有意義的差別。本研究得到的結論為，在有膀胱外阻塞症的病人中，年齡並不會有症狀積分、前列腺容量及膀胱功能有明顯的影響。在任何年齡層，只要有阻塞存在，阻塞的程度才是影響排尿障礙的主因。(慈濟醫學1996; 8: 197-203)

In order to understand whether age can influence the urodynamic expression in symptomatic bladder outlet obstruction, 418 patienets with sever prostatism were retrospectively enrolled in this study. All of them had an IPSS symptom score of more than 15 and none had diabetes or enurogenic disease. Transrectal songraphy of the prostate and uroflowmetry were routinely undertaken in them. Cystomerty was performed in 384, pressure flow study in 323, and urethral pressure profilomety in 202 patients. The patients were further divided into subgroups at a ten-years interval. The uroynamic parameters were compared among different age groups and regression analysis was made between age and flow expressions. The results of this study revealed that age had no significant impact on the urodynamic parameters in the patients with symptomatic bladder outlet obstruction. The severity of outlet obstruction and the bladder function in response to obstruction differed little in men of any ages as long as presence of bladder outlet obstruction.
