本研究旨在探討母親獲知新生兒為「先天性甲狀腺功能低下症篩檢陽性」時,所產生之情緒反應及對母親所造成的影響。研究對象選自台灣北部及南部及一家醫學中心新生兒甲狀腺功能低下症篩檢陽性之個案母親,共得43名個案。研究者以會談方式收集母親回溯性的主觀經驗,並以「內容分析法」分析會談資料。結果歸納出母親獲知篩檢陽性時之情緒反應有:擔心,震驚、害怕、緊張、慌亂、著急、,哭泣,情緒低落,心存疑慮,內疚、自責等;此篩檢結果對母親的影響則包括了對(1)母嬰互動、(2)日常生活、(3)生育計劃三方面的影響。藉由本研究之結果,可作為改善目前新生兒篩檢告知系統之參考,並使篩檢告知人員更深入瞭解母親之反應,給予適切之指導與解說,避免不必要之誤解與困擾,以利篩檢告知工作之進行。(慈濟醫學1996; 8: 231-237)
The purpose of study were to explore the emotional responses and influences of mothers when they were informed the positive newborn screening result of congenital hypothyroidism. Subjects were selected from two medical centers in the northern and southern Taiwan. Forty-three mothers were interviewed. The data were systematically analyzed, categorized and quantified by using “content analysis”. Results of this study revealed that these mothers showed eight types of emotional response: concern, shocked and fear, tense, worry and confusion, crying, emotional-down apprehension, remorse and self-reproach. The influences of these mothers were typed three aspects: mother-infant interaction, daily life, and reproductive plan. Based on the result, we can suggest the clinical workers how to notify the newborn screening results to mothers exactly, and they should have to understand the mothers’ emotional responses, influences and give them appropriate explanation.
