慢性腎衰竭患者的疾病認知、態度及自我照顯行為之研究

摘要

慢性腎衰竭威脅國人健康甚鉅，患者的疾病認知、態度及們我照顧行為，對於疾病控制相當重要。國外有關此方面的研究相當少見，國內尚缺乏文獻探討。本研究的目的在討慢性腎衰竭病人疾病認知、態度及自我照顧行為的現況及其影響因素。於某醫學中心腎臟科門診，以疾病認知、態度及自我照顧行為量表會談38位個案，進行資料收集。結果發現患者的認知偏低，尤其血清肌酸酐、高鉀水果、高品質蛋白質、代鹽製品認知最差，態度不偏正向負向，採中立看法，但隨血清肌酸酐值增加而趨負向。自我照顧行為尚好，但在危險因子警覺與預防方面仍差，研究對象有76%患者採用民俗療法。年齡、教育程度、血清肌酸酐值、參與腎臟課程以及接受營養師指導，分別影響患者的疾病認知、態度及自我照顧行為，本結果可提供慢性腎衰竭患者衛生教育及醫療照護之參考。(慈濟醫學1996; 8:313-321)

Chronic renal failure is common in Taiwan. Patient’s knowledge, attitude to disease and self-care behavior are important to prevent deterioration of renal function, but no studies covering all these aspects have been done. The aims of this study were to investigate the knowledge, attitude and self-care behavior of patients with chronic renal failure and to analyze the relationship among them. Thirty-eight patients with chronic renal failure were enrolled from the outpatient clinic of nephrology in a medical center. The patients were interviewed by using questionnaires for knowledge, attitude, and self-care behavior about chronic renal failure.
The results showed that patients with chronic renal failure had inadequate knowledge on disease. Their knowledge about the significance of serum creatinine level, high-potassium fruits, high-quality-protein food, and salt substitutes were the worst. Generally, patients hold neutral attitude toward their disease. Their attitude became negative as serum creatinin level increased. These patients had a fair self-care behaviors. Nevertheless, their behaviors on medical compliance, awareness and prevention of risk factors were poor. Seventy-six percent of our patients took herb medicine. The knowledge, attitude to disease and self-care behavior of patients with chronic renal failure were influenced by age, education level, serum creatinine level, attendance of health teaching program and nutrition counseling.
