以QBC®血液寄生蟲檢測法診斷瘧疾之效率

Efficiency of QBC® Blood Parasite Detection Method for the Diagnosis of Malaria

摘要


瘧患在投藥時,血中瘧原蟲濃度及其發育形態會產生變化。本研究在比較QBC血液寄生蟲檢測法和傳統血液厚層塗抹法,於低瘧原蟲血症時,對不同瘧原蟲型體的檢查效率。當血液瘧原蟲濃度低於100/Ul,QBC法對熱帶瘧指環體的感度為100%,血液厚層塗抹法為70%(n=10);反之,間日的阿米巴型體則以血液厚層塗抹法QBC法為佳(100% vs.64%,n=14)。熱帶瘧患者週邊血液中的瘧原蟲,以指環體占大多數(88.9%±15.0%,n=6),可見QBC法特別適合熱帶瘧的檢查。至於日瘧患者血液中,以阿米巴型體占多數,但大部分受檢者(7/8)都螱少也有指環體,應該亦可以QBC法檢測。因QBC法操作簡單、快速,如先以QBC法對大量的檢體作篩檢,陽性患者再進一步看血液抹片以確定瘧原蟲種類,不失為可行的辦法。(慈濟醫學1998;10:125-130)

During treatment of malaria patients, different developmental stages and densities of Plasmodium are present in blood. In this study , the efficiency of the QBC blood parasite detection method and conventional thick blood film examination were compared for the diagnosis of various stages of malaria parasites in patients with low parasitemia(<100/uL). In these cases, the sensitivity of the QBC method for P.falciparum ring form was 100%, and that of thick films was 70%(n=10). On the contrary, for P. vivax ameboid form detection thick film examination was preferable to the QBC method (100% vs. 64%,n=14). Rings were the majority form (88.9%±15.0%, n=6) in the peripheral blood of dpatients infected with P.falciparum, indicating that the QBC method is particularly suitable for the diagnosis of falciparum malaria. In vivax malaria infection, the majority of parasites in patient blood were ameboid forms with some percentages of rings detectable in most of the patients examined (7/8), suggesting that the QBC examination is also adequate for diagnosis of vivax malaria. With the speed and ease of operation, the QBC method should be practical for screening a large number of blood samples before identification of malaria species by blood films.
