Systemic Tuberculosis Initially Diagnosed by an Oral Lesion: A Case Report

經由口腔病灶之診斷而發現全身性結核病-病例報告

摘要


口腔結核病變是很少見的病變。本文報告一位三十四歲,因肛門膿瘺而住院接受治療,卻不知他已患有結核病的客家人,經由口腔病灶之病理組學診斷而後的得知已羅患全身性結核病的個案,其細菌並經由核酸聚合素連鎖反應檢測證實為桔核分枝桿菌。藉此以提醒我們,透過仔細的查檢及鑑別診斷,牙科醫師也可在結核病之早期斷與減低病患之接觸上,扮演一重要的角色。(慈濟醫學 2000;12:67-71)

Oral tuberculosis is rare. A 34-year-old Haaka man admitted for treatment of an anal fistula was not aware he had systemic tuberculosis. The detection of an oral tuberculous lesion histologically led to the identification of systemic disease, and the bacteria was further confirmed by PCR (Polymerase Chain Reaction ) assay as Mycobacteria tuberculosis. Through the cautious differential diagnosis of oral lesions, the dentist can play a role in the early detection of tuberculosis and reduce the risk of exposure. (Tzu Chi Med J 2000; 12:67-71)
