以急性眩暈表現之腦幹血管畸形 - 二例報告

Brainstem Vascular Malformation Presenting with Acute Vertigo - Report of Two Cases

摘要


血管畸形可發生於顱內各處,多以癲癇及局部神經學障礙為表現。如果發生腦幹,可能影響中樞前庭神經系統,而造成眩暈及異常眼球運動。最近我們驗兩例以眩暈為主訴的病人,病例一,女性60歲,出現單側性眼振;病例二,女性44歲,出現兩側性眼振,均在眼振電圖檢查上呈現多重異常的中樞性徵象。經核磁共振影像檢查,前者診斷為腦幹血管畸形,接受開顱手術切除,病理報告為靜脈血管瘤;後者診斷為腦幹海棉狀血管瘤,僅接受保守性藥物治療。二者之預後皆良好。

Vascular malformation can be found in various parts of the central nervous system and usually presents with headache, seizures, and focal neurological deficit. The central vestibular system may be involved and can manifest as vertigo with abnormal eye movements. Herein, we report on two patients presenting with acute vertigo with multiple central signs on electronystagmographic examination. MRI scan revealed brainstem angioma in patient 1, who underwent surgical intervention; patient 2 had cavernous angioma in the brainstem and received conservative treatment with pharmacotherapy only. Both patients survived and returned to a normal life.
