甲狀腺功能低下症相關之高泌乳素血症－二病例報告與文獻回顧

摘要

吾等報告在國立成功大學附設醫院內科部內分泌新陳代謝科所經歷之二例與甲狀腺功能低下症相關的高泌乳素血症案例。第一案例是甲狀腺切除術後導致的甲狀腺功能低下症併高泌乳素血症的女性病人，以甲狀腺素治療後，溢乳的症狀消失，並且電腦斷層檢查發現的腦下垂體微腺瘤也跟著消失。第二個案例則是對多巴胺激動劑頑抗的高泌乳素血症，單獨使用甲狀腺素治療次臨床之甲狀腺功能低下症，顯然不足以降低血清泌乳素值。我們回顧甲狀腺功能低下症併相關的高泌乳素血症相關文獻，並討論目前對這一特殊臨床狀況的各家看法。

We report on 2 cases of hyperprolactinemia associated with thyroid dysfunction. The changes in clinical symptoms and serum prolactin levels highly corresponded to thyroxine therapy. The first case had galactorrhea associated with overt hypothyroidism and pituitary microadenoma. All abnormalities disappeared after thyroxine therapy. The second case had galactorrhea resistant to dopamine agonist therapy and demonstrated improvement after thyroxine replacement. Hyperprolactinemia recurred after withdrawal of the dopamine agonist. We review the literature on hypothyroidism-related hyperprolactinemia and discuss the current opinions about this condition.
