慈濟醫學中心醫療影像儲存與傳輸系統(PACS)之建置與效益評估

The Establishment and Effectiveness of the Picture Archiving and Communication System (PACS) at Tzu Chi Medical Center

摘要


介紹花蓮慈濟醫學中心醫療影像儲存與傳輸系統(PACS)的建置與效益評估。材料與方法:(1)PACS的建置結構:經由三種方式自院內放射科各檢查儀器捐擷取影像,所有擷取的影像以DICOM方式傳入放射科之影像伺服器主構,進行資料的儲存與管理。這些影像再經由科內的10部品管工作站進行影像的品質管制與資料備份,這些經品管後的影像皆存放在影像伺服器主機,再以高效率網路傳輸到各診間及病房,以四螢幕或螢幕的方式呈現品管後的檢查影像。(2)統計病患到本院放射科進行檢查到影像取得所需時間,比較無片化實施前及實施後,本院診療效率的改變情形。(3)統計本院X光片使用量、處理因洗X光片産生的廢棄物的改變情形,探討無片化實施前後的環保效益。結果:PACS實施後,病患在一般攝影檢查所需的等候時間爲傳統管理作業流程的二分之一以下;本院每年減少一萬五千公升以上的廢水污染源,節省了龐大的醫療成本。結論:無片化後的效益(1)提昇診療效率,減少病患等候時間。(2)精確有效的影像資料管理。(3)減少資料儲存空間。(4)各式軟片耗材及自動洗片等週邊設備成本降低。(5)降低污染及符合環保。(6)遠距醫療及跨區會診之便利性。

Objectives: To introduce and evaluate the effectiveness of the PACS at Buddhist Hualien Tzu Chi Medical Center. Materials and Methods: (l)The PACS was constructed as follows: images captured from imaging machines in the radiology department were transferred into the main server by way of DICOM (Digital Imaging and Communication in Medicine). The servers were used for storage and organization of captured images. Images were then processed and permanently stored by the quality control stations of the radiology department. The qualified diagnostic images could then be transferred to the ward and out-patient department through a four-view or single-view monitor. (2) The effectiveness of the radiology department before and after PACS construction was evaluated base on the time required to access images after radiological examination. (3) The role of PACS in environmental protection evaluated based on reduction of film-based imaging. Results: The time required to access images after radiological examination was markedly reduced after the initiation of PACS. The amount of pollution generated by materials from film processing was also markedly reduced. Conclusion: The PACS is effective based on the following factors: (1) Increased medical efficiency, with reduction in waiting time for diagnostic imaging results. (2) More efficient for administration of imaging data. (3) Decreased space for storing images. (4)Reduced processing of film, leading to reduce the medical costs. (5) Environmentally friendly. (6) Establishment of tele-communication and tele-consultation.
