A case of retroperitoneal hemorrhage due to spontaneous rupture of a left adrenal pheochromocytoma presented as acute abdominal emergency with symptoms of hypovolemic shock. An elective operation was successfully performed 4 months later under correct diagnosis and sufficient preoperative preparation including volume replacement and pre-operative administration of α-adrenergic blocking agent.
