The Role of Hematuria in Predicting the Outcome of Severe Acute Respiratory Syndrome

血尿在預測嚴重急性呼吸道症候群預後的角色

研究目的:本篇在探討嚴重急性呼吸道症候群在泌尿道的影響,以及尿液分析結果與嚴重急性呼吸道症候群預後的關係。 材料與方法:自2003年3月26日到2003年6月13日,共有68位符合世界衛生組織(WHO)定義的嚴重急性呼吸道症候群病人住在本院接受治療而納入本研究。病人住院過程之中,如果死亡或是接受呼吸道插管治療者定義為嚴重疾病組,其他的病人則定義為輕微疾病組。病人的基本資料、臨床表現以及最初的尿液分析結果皆予以紀錄,並以單變相以及多變相分析比較兩組之間的差異。 結果:在這68位嚴重急性呼吸道症候群病人當中,22位(32.4%)為醫護工作人員,35位(51.5%)是由其他醫院轉診至本院接受治療者。糖尿、蛋白尿、血尿以及膿尿分別出現在6 (8.8%)、23 (33.8%)、9 (13.2%)以及8位(11.8%)病人身上。單變相分析結果顯示,較大的年紀、轉診來的病人、尿液的pH值偏酸、蛋白尿以及血尿皆與嚴重疾病組有顯著的關係。然而,多變相分析結果顯示,血尿是嚴重疾病組唯一的預測因子(勝算比[O.R.]11.250;95%信賴區間2.092到60.485;p = 0.005)。 結論:本研究結果顯示,血尿可能是嚴重急性呼吸道症候群臨床預後的預測因子。

Objective: We investigated the impact of severe acute respiratory syndrome (SARS) on urological manifestations, and the association of results of urinalysis with clinical outcome of SARS. Materials and Methods: In total, 68 patients admitted to Taipei Veterans General Hospital from March 26 to June 13, 2003 with a diagnosis of probable SARS according to the World Health Organization (WHO) definition were enrolled in this study. Patients who died or were intubated with mechanical ventilation were considered to have major illness, with the others considered to have minor illness. Demographic information, serial clinical manifestations, and initial voided urinalysis data were analyzed. Univariate and multivariate analyses were performed. Results: Of the 68 patients, 22 (32.4%) were healthcare workers, and 35 (51.5%) were referred from other hospitals. Glucosuria, proteinuria, hematuria, and pyuria were noted in 6 (8.8%), 23 (33.8%), 9 (13.2%), and 8 patients (11.8%), respectively. Univariate analyses showed that advanced age, referred patients, low urinary pH, proteinuria, and hematuria were significantly associated with major illness. However, by multivariate analyses, hematuria was the only predictive factor for major illness (odds ratio, 11.250; 95% confidence interval, 2.092 to 64.485; p = 0.005). Conclusions: The results of this study suggest that initial voided hematuria may be a significant predictor of major illness with SARS.
