Simultaneous Treatment of a Renal Stone and Nephroptosis with a Single Session of Percutaneous Nephrolithotomy and Nephropexy: A Case Report

摘要

在台灣泌尿道結石是一種常見的疾病，大多數的病例，選用內視鏡手術(endourological surgery)或體外震波碎石術(ESWL)來治療都可以得到很好的治療結果。然而，有些時候泌尿道同時存在的病理狀況可能使得結石的處理變的較為複雜及困難，或者同時需要其他的手術來處理這些狀況。我們在一位同時有腎結石及游離腎的患者身上，同時施行經皮穿腎取石術及腎臟固定術，手術順利而且成功。在經過一年的追蹤之後，這位患者已無結石，腎臟也已經固定在較高的位置，病人原本腰痛的症狀也完全緩解。經皮穿腎腎臟固定術利用腎造廔路徑與周邊組織的沾黏可以成功的將腎臟固定於較高的位置，不但手術時間短，而且較為經濟。此外，更可以同時施行其他的經皮穿腎手術。在長達一年的追蹤，我們有很好的手術結果，今後我們還需要更長時間的追蹤及累積更多的經驗。

Urolithiasis is a common disease in Taiwan. [1] In most cases, both endourological surgery and extracorporeal shock wave lithotripsy (ESWL) are feasible interventions. [2] However, concomitant urologic conditions may complicate the management of urolithiasis, and necessitate additional procedures. We report a patient who presented with a right ureteropelvic junction stone and significant right nephroptosis. We successfully treated the patient with percutaneous nephrolithotomy and percutaneous nephropexy in a single session.
