中醫“辨證”相關用語之語義探討
On the Semantic Analysis of Terminologies Used in "Pattern Identification" of Traditional Medicine
摘要
中西醫之臨床合作及研究已進行多年，但從目前之成果來看仍未盡如意，甚至中西醫師在溝通及建立共識上也多困難。究其原因，除了兩套醫學在理論體系本身確然存在著不同之觀點外，最核心的原因仍是語言問題。為克服相關問題，必須進一步釐清中西醫的許多相關概念。本文藉由語義學與科學哲學中「典範」的觀念與方法，逐一探討了中醫「辨證」一詞及現代醫學中的類似用詞，兩者間的概念異同。本文研究成果，可協助解決中西醫合作時的溝通語言問題，了解彼此語言的運用範疇，促進兩者之整合。另一方面也發現「證」或「證型」皆具有表述藥物功效、機轉的功能，可用於新藥研究發展上說明藥物之功效或機轉。

Attempts to integrate Chinese and western medicine both in clinical practice and in research have been underway for almost 5 decades. The results seem to be unsatisfactory as there exists huge communication gap and lack of consensus for practitioners from both sides. The reason is not only because of the dichotomy of the fundamentals of both medicines but the language and terminology per se. To overcome this, it is vital to clarify various relevant concepts in both medicines. Using the concept and method of paradigm from semantics and philosophy of science, this article tries to clarify and distinguish the difference and meaning of some ”pattern identification” terminologies used by both disciplines. Our findings can be used to understand the meanings of various common terms and solve the language problem in integrating both medicines. Furthermore, we concluded that both ”pattern (Zheng)” and ”Zheng Xing” have the functionalities of representing the effect and mechanism of medicines, which can be used upon R&D of new medicines to explain the effect and mechanism of medicines.
