腹軸動脈分支形態之血管影像評估

EVALUATION OF CELIAC AXIAL BRANCHING PATTERNS IN ANGIOGRAPHIC STUDIES

摘要


腹軸動脈攝影術(celiac angiography)對於上腹部內臟疾病之診斷非常重要,但在正常情況下,腹腔動脈及其分支,當有各種不同程度的變異(variations),使得檢查操作和X光判讀發生困難。三軍總醫院放射科自民國59年至67年間,軍方男性住院病人經病理檢査,證實爲原發性肝癌(primary liver cell carcinoma),且做過血管攝影檢查,資料完整的共有91例,其年齡在19歲到69歲間。本文僅對其血管攝影結果,有關腹軸動脈分支形態加以整理,分析與討論。

Celiac angiography is very important for the diagnosis of diseases in the upper abdomen. Normal variations in the celiac axial branching patterns are frequently encountered. For the execution and interpretation of visceral arteriograms, the radiologist requires a thorough knowledge of normal and varient vascular anatomy. In this presentation we analyse the angiographic findings of the celiac axial branching parterns of 91 cases of pathologic verified primary liver cell carcinoma collected in our hospital from 1970 to 1978. All are male. Comparison of the variations with other literature is discussed. Eighty-two cases (90.1%) with the normal celiac branching patterns of left gastric, splenic and common hepatic artery. Most of the left gastric artery are arised from celiac trunk (91%) with other variations from common hepatic artery, splenic artery and left hepatic artery. Majority of the blood supplied to the liver are derived from common hepatic artery (91.2%) which were arised from celiac axis in 76 cases (83.5%) and 3 cases (3.3%) originate from superior mesenteric artery. Variations of splenic artery are infrequent, most of them (94.4%) were arised from celiac trunk. The marked variability in the origin of the major celiac arterial branches are extremely important to the angiographor both for the performance of a complete examination and for correct interpretation. It is also important for the surgeon as a guide to operation in the stomach, liver, pancreas or gall bladder. The knowledge of vascular anatomy with variable varients is basic and very important to every radiologist and surgenon.
