THALAMIC HEMORRHAGE AND VENTRICULAR BLOODY CASTING～C-T scan diagnosis and literature review-Analysis of 84 cases of thalamic hemorrhage in Chinese

In studying 2,500 cases of head C-T scan of Chinese within one year (from July 1979 to July 1980), we found 84 cases (3.4% against total 2,500 cases) of non-traumatic thalamic hemorrhage and 233 cases (9.3% against total 2,500 cases) of non-traumatic intracerebral hemorrhage. 84 cases of non-traumatic thalamic hemorrhage out of 233 cases of non-traumatic intracerebral hemorrhage, the percentage is 36%. The most important complication of in-tracerebral hemorrhage is thought to be the ventricular bloody casting (VBC) in our series of study of intracerebral hemorrhage in Chinese, furthermore, we found 122 VBC cases out of 233 cases of non-traumatic intracerebral hemorrhage, while 66 VBC cases out of 84 cases of non-traumatic thalamic hemorrhage and 18 cases non-VBC. Ventricular bloody casting is thought to be the cause of hydrocephalus that induced mortality finally. In comparison of hydrocephalus between VBC and non-VBC cases, we found 46 cases (85%) of hydrocephalus in above mentioned 66 VBC cases and 8 cases (15%) in 18 non-VBC cases, but among the 8 cases of hydrocephalus, 5 cases were due to senile cerebral atrophy of mixed type of cortical and central atrophies. The mortality rate was very high- 55% (46 cases out of 84 cases); 42 in VBC and 4 in non-VBC cases, among the 42 cases of mortality in VBC, 40 cases complicated with hydrocephalus. The author analyzed 84 cases of thalamic hemorrhage in Chinese, from the points of views in anatomy, physiology, pathology, clinical course and prognosis. The complication of hydrocephalus caused by VBC paid a great role in the causes of death.
