Billiary ascariasis is almost worldwide distributed. The adult worms mainly lodge in the ileum. It is probable that the adult worms potentially migrate into the common bile suct, gallbladder and/or intraphepatic ducts. We recently experienced a case of gallbladder ascariasis diagnosed by ultrasound which, to our knowledge, was so far rarely reported in the literatures. A 55 year-old female patient, with complaint of intermittent dull apin over the. right upper quadrant of abdomen for years, had no tenderness in the epigastrium and RUQ. Her liver function tests exhibited no significant change. An IVC revealed vermiform filling defect in the gallbladder. Real-time abdominal echograms showed a thick coiled echogenic strip with the central anechoic ”inner tube” sign. On cholecystectomy a long coiled wormlike stone, the same figure as shown in the abdominal echograms, was found. The stone was fragile and divided into 3 fragments when it was taken out of the gallbladder.
