醫用超音波對診斷腹部腫塊之應用

Application of Ultrasound in Abdominal Masses

摘要


榮民總醫院放射線部自民國六十八年八月中旬引進新型灰色尺度超音波機器後,在最初六個月期間共對疑有腹部腫塊之息者127人作詳細檢查;其中有15人診斷為正常,經六個月追踪後仍無異狀發現;46人有腹內各種器官之病灶;其餘66人失去追踪。在46例陽性診斷之病患中,其腫瘤生長之部位包括以下各系統:淋巴系統、肝膽系統、胰臟、脾臟、腎臟、生殖系統、消化系統等。其中19例經外科及病理證實。病灶之超音波影像可分為三種型式,即囊狀、固質或混合型。可依其內部回音之有無,病灶壁之鮮明度和音束有否減弱等等而決定。以吾等之經驗,超音波對腹部之單純囊腫、感染囊腫、壞死或固質之腫瘤等有甚高之診斷率,以其方便施行,無放射線傷害之虞,及可同時對多個器官偵察而言,實可作為懷疑腹部腫塊之第一線檢查。

127 patients suspected to have abdominal masses had been examined by the gray-scale ultrasound within six months since August 1979. 15 patients was diagnosed as normal and follow-up for six months without showing any abnormality. 66 patients lost of follow-up. 46 patients were diagnosed as having various kinds of intra-abdominal lesions involving different systems. They included the followings: lymphatic system, hepatobiliary system, pancreas, spleen, renal, reproductive system and gastrointestinal system. 39 cases received surgical intervention and proved by pathology. 7 cases were supported clinically. The sonographic appearance can be divided into 3 groups, namely, cystic, solid or mixed. They are grouped according to whether there is presence of internal echoes, the distinct boundary of the lesion and the presence of attenuation of the sound beam. According to our experience, ultrasound is useful in the differentiation of simple cyst, infected cyst, solid tumors or presence of necrosis inside the tumor. Ultrasound should be recommended as the first examination for patients with suspicion of abdominal masses since its diagnostic rate in high, convenient to carry out, no radiation ionization, and can be used for multi-system simultaneously.
