Preliminary Study of Alternative Tomographic System for Radiotherapy Planning

取代性電腦斷層放射治療計畫之初步研究

摘要


電腦斷層攝影為和庫上一項偉大的發明,它已經在放射診斷領域中占有極重要的角色。但是電腦斷層攝影可為放射腫瘤學提供那些幫助呢?其中最重要的一項當是電腦斷層攝影機運用在放射線治療計畫中。因為電腦斷層影像不但可精確地描繪出腫瘤的位置,而且能對此一腫瘤橫切面上不同組織密度提供細微的資料,這些都是對提升某些可治癒瘤病放射治療計畫中劑量準確分佈所不可缺少的要件。然而因電腦斷層攝影花費太高且需求過量,使得其真正被運用在經常性可癒癌病腫瘤定位上發生極大困難。在這種情況之下,國外少許研究機構設法利用已有的放射治療設備如放射治療計畫電腦、同心旋轉的模擬攝影機、同位素治療機或直綫加速器,經適當努力已成功地轉換成取代性電腦斷層攝影系統。在本篇初步研究報告中,理論與實驗兩部份都做了探討,以為將來發展實際取代性電腦斷層放射治療計畫系統做準備。

Computerized axial tomography (CT) is a marvelous scientific invention. It has well been established on the realm of diagnostic radiology. But what can CT devote its help to radiation oncology? One of the leading topics should be the CT scanner used in radiotherapy treatment planning. Because CT image can tell not only the precise location of tumour, but also the very subtle differences in tissue heterogeneities on the same tumour level of body cross section. Those information is essential to increase the accuracy in dose delivery for some curative radiotherapy treatment planning. However, the high cost and large requisition for CT scanning make the situation too difficult to do routine curable cancer patient's tumour localization. Under this circumstances, the. idea of linking the available facility such as treatment planning computer, isocentrical simulator as well as isotope machine or linear accelerator together with some efforts in order to serve as an alternative tomographic system have already been carried out successfully by several foreign institutes. In this preliminary study, both the theory and the real practical test were done in order to prepare for further development of a true alternative tomographic system for radiotherapy planning.
