摘要

脂肪脊髓脊膜膨出是一種罕見的先天異常，源於脊髓及脊膜經由脊柱裂而向外突出。榮民總醫院自1980年至1983年6月共有10個病例，男女各5人，年齡自2個月大至27歲，平均年齡9.5歲。腰背部組織突出，下肢運動障礙及大小便失禁為其重要臨床主訴；而異位脂肪瘤及脊索拘繫則為致病的基本原因。放射線學檢查則以腰薦椎常規攝影、脊髓攝影和電腦斷層檢查為確定診斷的最佳工具。本文根據此10病例，就臨床表徵、病因學及放射學檢查所見，提出報告和討論。

Lipomyelomeningocele is an uncommon congenital anomaly, resulting from outpouching of spinal cord and its surrounding leptomeninges through the bony defect of spina bifida. Ten cases were found at VGH from 1980 to June of 1983, including 5 males and 5 females between 2 months and 27 years of age, the medium age was 9.5 years. Protruding mass of lumbosacral region, motor dysfunction of lower extremity and disturbance of sphincter function of urination or defecation were chief complaints of these cases; lipoma with tethered spinal cord was the basic structural anomaly which caused clinical problems. Radiologic evaluation including lumbosacral routine, myelography and computed tomography was the modality of choice for confirmatory diagnosis.
Clinical manifestations, radiologic findings were analyzed, and pathogenesis was discussed.
