摘要


榮民總醫院放射線部自民國70年2月至71年12月間,共有206名患有乳房腫塊之婦女,使用U.I. Octoson超音波儀作乳房掃描。其中有33名經病理證實爲乳癌,複閲此33例之超音波圖形,依據下列四項原則加以分析:(一)腫瘤之形狀及邊緣。(二)腫瘤之内部回音。(三)腫瘤背後之音波減弱效應。(四)腫瘤周圍之變化情形。其中82%具典型之惡性腫瘤超音波圖形:邊緣不規則、內部呈不均勻之弱回音、腫瘤後方有顯著之音波減弱效應以及各種周圍組織之惡性徵象,如:皮膚增厚、皮膚與乳頭之凹陷、腫瘤周圍之線狀強回音、胸壁受腫瘤侵犯及腋下淋巴腺腫大等。本文並提出超音波在乳癌診斷上之優劣點及其鑑別診斷。

During the two year-period from Feb, 1981 to Dec. 1982, 206 cases with palpable breast masses were scanned by using U.I. Octoson, There were 33 cases subsequently proved to be of malignant nature pathologically. All the imaged mass lesions were analysed retrospectively according to the following 4 criteria: (1) shape and margin (2) internal echoes (3) attenuation effect (4) associated findings. 82% of these cases showed characteristic echo patterns including: irregularly marginated mass with dense linear radiating echoes, inhomogenous and weak internal echo, great sound attenuation and some malignant signs in the surrounding tissue. The superiority and limitation of ultrasound diagnosis in breast malignancies and their differential diagnosis were discussed. When a breast mass is encountered, excluding malignancy is most important to every physician. In our experience, ultrasound mammagraphy provides high accuracy in differentiation between malignant and benign breast lesions.
