Carcinoembryonic Antigen (CEA) in Colorectal Cancer

Pre-operatively, carcinoembryonic antigen (CEA) titers were determined in 145 proven colorectal CA. 86 cases (59.3%) presented abnormal results in the tests. The assay was abnormal in 33.3% with Dukes' A of colorectal CA, 31.4% with Dukes' B, 57.5% with Dukes' C and 97.8% with Dukes' D. The abnormal rate of CEA on the right sided of the colon is higher than that on the left. Pre-operatively, 45 cases (31%) of all patients had distal metastases (38 cases with liver metastasis, 8 cases with lung metastasis and bone metastasis 3 cases). Markedly increased serum CEA titers (over 25ng/ml) strongly suggests liver metastases in colorectal CA. Tumor resectable rate depends on the serum CEA values, curative resection can be done in normal CEA value, incomplets resection was 7.7% with serum CEA level within 3.5-10 ng/ml, 11.8% within 10-25 ng/ml and 58.1% with CEA values over 25 ng/ml. Pre-operatively 86 cases presented abnormal CEA level, 53 cases gave post-operative CEA levels as follows: (1): one month after surgical treatment, 38 cases showed signficant decrease in CEA titer and (2): 24 cases/38 cases, returned to normal limits. Determination of CEA titer often is helpful in evaluation of prognosis, recurrence and metastasis in patients with colonic cancer.
