右肺中下葉合併塌陷之胸部攝影所見

摘要

多肺葉合併塌陷之胸部X光攝影所見在過去文獻僅有簡略敍述。本院最近發現4例右肺中下葉合併塌陷之病例，若僅用過去文獻所述難以做解剖學之診斷。在此4例中我們發現一些過去文獻所未記載之X光徵象；即右肺門區之支氣管與右主支氣管喪失直線銜接關係或雖成直線銜接但與氣管成寬角度銜接，葉間動脈(interlobar artery)、中間支氣管(intermediate bronchus)及右下肺野水平走向之肺靜脈消失不見，這些所見可由正常解剖關係得到合理的解釋。我們相信這些所見較過去文獻所述容易辨認而有助於右肺中下葉合併塌陷之解剖學診斷。

Plain film findings of combined right middle and lower lobes collapse have not been emphasized in the literature. Recently, we found four such cases. Using the X-ray signs described in the literature alone, we have difficulty to make a correct anatomical diagnosis. In these cases, some constant findings were found, those are abnormal angulation and/or connection between right hilar bronchus, right main bronchus and trachea, nonvisualization or disappearance of interlobar artery, intermediate bronchus and horizontally oriented pulmonary veins in right lower lung field. These findings can be easily explained by normal anatomy. We believe these findings are specific for the combined right middle and lower lobes collapse and can be very helpful to make an anatomical diagnosis.
