電腦斷層引導對脊椎病變之針取式活組織檢查法

摘要


電腦斷層攝影可提供病灶與其鄰近器官解剖學三度空間關係的影像,因而可利用於引導針取式活組織檢查,提高對深部位病變針取式活組織檢查的安全性及準確性。我們曾對15位病例的17處脊椎病變進行電腦斷層引導針取式活組織檢查,其中5處在胸椎、11處在腰椎及1處在薦椎,年齡分布為三歲至七十八歲。病理學陽性診斷率為94%(16/17),分別是慢性骨髓炎、結核病、轉移性腺癌、慢性炎症、纖維化、神經膜瘤、嗜伊紅性白血球性肉芽腫及內生軟骨瘤。檢體微生物培養陽性率為56%(5/9),分別是結核桿菌及葡萄球菌。總計17次的電腦斷層引導針取式活組織檢查均未發生併發症。

Computed tomography can provide a precise three dimensional anatomic image of the lesion and its surrounding organs. It can be used as a guidance in percutaneous needle biopsies to increase the pathological accurate rate and to minimize the complications. We had performed 17 CT-guided percutaneous needle biopsies on 15 cases from 3 to 78 years of age. There were 5 on thoracic vertebra, 11 on lumbar vertebra and 1 on sacrum. Chronic osteomye-litis, tuberculosis, metastatic adenocarcinoma, chronic inflammation, fibrosis, neurilemmoma. eosinophilic granuloma and enchondroma were reported pathologically with a 94% (16/17) positive rate. Tubercle becilli and staphylococcus were yielded in the bacterial culture studies with a 56%(5/9) positive rate. There were no procedure related complications.
