腦部動靜脈畸型之電腦斷層影像

Computed Tomographic Findings of Intracranial Arteriovenous Malformations

摘要


67例腦部動靜脈畸型(arteriovenous malformations,以下簡稱AVM)病例經回溯統計其電腦斷層攝影(computed tomography,以下簡稱CT影像)。其中男性30例、女性37例,年齡以11至30歲最多(63%), CT影像呈現急性出血佔38例(57%),以腦實質內出血最多。未出血之病例中,腫塊效應出現於14例(20%),注對比劑前之影像,正常者11例(38%),病灶呈高吸收值區者15例(52%)呈似水囊狀低吸收區3例。二例硬模(dural)AVM注對比劑前後影像均正常。出血病例中,6例在注對比劑後呈不正常顯影增強(enhancement)。67例中,硬膜AVM有5例,其中2例引流至海棉寶(cavernous sinus)。有二例病灶發生於側腦室內脈絡叢(choroids plexus),而以單側腦室內出血表現。3例血管攝影陰性均合併腦實質內出血。二例合併動脈瘤(aneurysm)。一例合併眼眶及面部血管瘤。

Intracranial arteriovenous malformation (AVM) is the most common intracranial vascular malformation, and is the most significant one clinically. 80% of the patients with intracranial AVM will present symptoms before the age of 50, with 2-3% chance to bear a bleeding episode every year, and 10% will die in each episode. From the clinical point of view, it is extremely important to diagnose and treat this entity early. Computed tomography (CT) has made it possible in recent years. We retrospectively reviewed the CT findings of 68 cases of surgically and pathologically proved intracranial AVMs. The results will be discussed here.
